Hip replacement surgery is one among the oldest surgeries in medicine. The first successful hip replacement was first performed in the year 1960.
Since then, the improvements in techniques and discovery of newer prosthetics have greatly improved the effectiveness of the surgical procedure
A hip replacement surgery is usually recommended for patients when.
1. Hip pain is so severe that day to day activities are grossly limited.
2. Pain is present even during the resting state, be it night or day
3. Stiffness in the joint drastically limits your ability to even move your leg
4. Hip pain does not respond to conventional pain medications or physiotherapy.
In general recommendations for hip replacement surgery is based on patients general pain and disability, not necessarily the age.
Most patients undergo hip replacements from between 50 to 80 years of age in general.
In general there are numerous clinical conditions that can lead to chronic long standing hip pain. These are namely
1. Osteoarthritis: In this condition, there is an age related wear and tear of the joint surfaces. This condition usually occurs with increasing age and also in those with a family history of arthritis. In this condition there is a systematic wear and tear of the cartilage that surrounds the end of the bones that form the hip joint.
2. Rheumatoid arthritis: This condition is autoimmune in nature were the body attacks its own system. In this conditions, the synovial membrane can get thick leading to severe pain and stiffness.
3. Post traumatic Arthritis: In this condition, a past serious injury to the hip or a past fracture can damage the cartilage leading to chronic ling standing pain and stiffness over a long period of time. Moreover, in some injuries to the hip, a dislocation or fracture may affect the supply of blood to the femoral head. This is a condition called Avascular Necrosis. In this condition, the surface of the bones collapse leading to severe arthritis.
4. Hip Disease in Children: Sometimes, children suffer from hip problems. While these issues might have been treated during childhood, these children may continue to suffer from arthritis much later in life.
Your ortho doctor will evaluate you with
1. Medical History – An in depth medical history is usually elicited for all patients planning for undergoing a hip replacement. General questions on health and assessment of your hip pain and its impact on your day to day lifestyle activities is usually done
2. Physical Exam – A full-fledged physical exam is done is then done to assess hip strength, mobility and alignment
3.Imaging – The last step of course is imaging with X-Rays or MRI that has to be done for all patients to assess the condition of the bones/soft tissues
Hip replacement surgery is usually done under spinal Anaesthesia. The actual procedure involves replacing the ball & socket of the hip joint with 2 basic components.
The first is a ball component that is made of metal or ceramic material and the second component is the socket made from a durable plastic cup, metal or ceramic.
The prosthetic components is placed in a way were it ‘press fits’ into the bone or in some cases may be cemented into place by your orthopedic doctor.
The choice of prosthesis of course depends on your bone strength, age and a host of other factors. The final decision rests with your ortho consultant.
The actual procedure takes only a couple of hours. Once done, you will be shifted into recovery at the earliest.
A large number of patients report an immediate lowering of pain and discomfort post a hip replacement surgery.
For proper recovery, patients must strictly follow all the instructions of the treating ortho doctor. Activity usually includes
1. A step by step walking program, that gradually improves mobility both at home and later
2. Specific physiotherapy and exercises that are done day to day to systematically improve joint strength and movement.
You should be able to resume all normal household activities within 6 weeks post the procedure.
You must follow with all post surgical recommendations for a healthy recovery from the procedure.
Some warning signs to take note are
Tenderness or pain in the calf
Shortness of the breath
Both of the above mentioned symptoms require immediate emergency intervention without delay.
SOME QUICK TIPS TO PREPARE YOUR HOME
Preparing your home to support your recovery post surgery is critical since it significantly lowers the risk of injury and speeds up your recovery process
Some of these are
Handrails in the shower or bath
Handrails in stairways
Elevated toilet seat with hand rails at an arms distance to get up
Firm cushioning to ensure that your knees are always above your hip level
